​Students with Active Role Models (SWARM)

 

The SWARM (Students with Active Role Models) program and Go365 reward teachers and staff for participating in physical activity with students throughout the school year.

Healthier teachers lead to healthier students and tracking and submitting your Points is simple! 

To qualify, you and at least one student must track at least 10 weekly physical activity events throughout the semester.  Use the SWARM tracking tool (included in the flyer below) to keep track of what events  you complete and your semester total!

Submission for Semester Points

Click on the link below to complete the SWARM submission about your activity in the SWARM program.  You will need your legal first and last name (as it appears on your Go365 ID card), date of birth, gender, Go365/HumanaVitality ID number (11 digits), and email address for the submission semester to receive credit.

Note: SWARM activity is subject to the Go365 Employer-sponsored Event Points limit of 700 per program year.  Looking for your Points? Check your Go365 statement for an Employer-sponsored Event valued at 350 Points on the last day of the month in which you submitted. The Points will be on your statement within 15 days after the end of the month in which you submitted. You have 90 days from the end of the semester to submit for Points.
