The KEHP is very proud of the partnership we have with the health departments. Thank you for your commitment in promoting health and wellness in your communities!

Below you will find information and resources to help facilitate onsite biometric screenings.

Why the LivingWell Promise?

The LivingWell Promise was designed to help members become more aware of their health status and to assist in improving the health of KEHP members as well as all Kentuckians.  Kentucky ranks 44th among all states in major health status categories like obesity, smoking, and diabetes.

How is Vitality Check part of the LivingWell Promise?

A biometric screening is an effective tool to educate and motivate KEHP members to make healthy lifestyle changes by creating an awareness and baseline understanding of their current health.
All LivingWell Promise planholders must complete their Health Assessment or biometric screening between January 1st and July 1st to fulfill their LivingWell Promise.  For more information on the LivingWell Promise, click here.  https://livingwell.ky.gov/Pages/LivingWellPromise.aspx

KEHP Member Biometric Screening Results and Consent Form
This document is kept by the health department and used to capture member’s results from their screening as well as their consent to have the screening.

(Note: You may add items to the form for the HD’s benefit, (i.e. the Health Department logo, etc.) however, no portion of this document may be deleted and language may not be changed.  Additional changes may cause potential billing or member issues.)

Health Department Event Promotion Form
If you are hosting an event open to all KEHP community members (ie: Saturday hours, walk-in days) the KEHP can send an email to invite all KEHP members in the area. Complete this form and send to your Engagement Consultant. A 4 week lead time prior to event date is required.
